Eligibility Requirements

To qualify for our day support program, individuals must:
✔ Be 18 years or older
✔ Have a diagnosis of Intellectual Disability/Developmental Disability (ID/DD)
✔ Be eligible for Medicaid waiver services (ID/MR Waiver, Building Independence Waiver, Family & Individual Supports Waiver, Community Living Waiver)
✔ Meet the program description requirements
✔ Have no significant medical conditions requiring invasive procedures

Discharge Criteria

An individual may be discharged if:
❌ They no longer require services
❌ Their behavior disrupts the learning environment or endangers others
❌ They fail to follow program rules or participate
